Three programs designed to give families clarity, calm, and a coordinated path forward — before a crisis forces the decision.
Qualifying families receive Housing Decision Coordination at reduced or no cost based on financial need. This is the same service provided to full-fee clients, covering family coordination meetings, communication facilitation, housing-transition organization, professional coordination, and a written transition roadmap.
Qualifying families receive Moving and Transition Coordination at reduced or no cost based on financial need, covering downsizing coordination, move logistics, vendor coordination, personal-property organization, and family communication support throughout the transition.
We train and educate long-term care providers, hospice organizations, estate planning attorneys, and financial professionals so they can help families create a coordinated plan before a health crisis forces the decision. Workshops are free to nonprofits, hospice organizations, and family support programs. For commercial professional organizations, workshops are available at $2,250 through LPA LLC — and all LPA LLC workshop proceeds support the Foundation's free community programs.
